Doctor Strange’s Zealots had a very identifiable look in the final film, but originally they looked vastly different.
Concept Artist Jerad S. Marantz worked on Marvel’s Doctor Strange and released some early drafts of the Dormammu worshipping villains. In the final version the Zealots, including lead villain Kaecilius, represent their newfound powers with a magical effect around their eyes. It almost appears as if their skin is breaking to reveal another person inside, but the original concept had a different way of showing it.

As you can see below, the previous version shows the dedication to Dormammu through the skin as well, but here the worshippers are given a two palette skin tone. Certain parts of the skin are left their normal hue, while other parts are brightened to give the face an almost tattooed or helmeted look. You can see a before and after version in the image below.

The newer version prevailed at some point, most likely because it gave all of the Zealots a look more reminiscent of the magical entity they serve.
